Job summary

Royal Bolton Foundation Trust are offering a fantastic training/apprenticeship opportunity to become an Acute Frailty Advanced Clinical Practitioner in partnership with the University of Bolton who deliver the 2 year MSc in Advanced Clinical Practice Apprentice Programme.
  • Applicants will be required to attend an interview and will be asked to provide a portfolio of work/evidence in support of their application. This may include undertaking preliminary assessments set by the University to enable us to assess their current academic ability and personal suitability for the course.
  • Applicants should also have GCSEs at grade C or above or grade 4 to 9 (or equivalent) in English and Mathematics. Please contact us for a list of acceptable GCSE equivalents.
  • Applicants are expected to have successfully completed an honours degree (or equivalent) in a relevant subject and have appropriate work experience for a minimum of 5 years.
  • The University of Bolton recognises prior learning, whether gained through academic credit and qualifications or through professional and employment experience. Previous learning may be used towards meeting the entry requirements for this programme or for exemption from part of this programme. It is the applicant's responsibility to make a claim for recognition of prior learning, and each claim will be considered in accordance with the University's policy and regulations for the recognition of prior learning.


Main duties of the job

  • Admission to this programme is only available to candidates who meet the following criteria. 1) They are employed for a minimum of 30 hours per week in a role relevant to the apprenticeship. 2) They have the right to live and work in the UK. 3) Their employer is participating in the Integrated Degree Apprenticeship scheme. 4) Their employer is willing to sponsor them to attend the University for two days per week (times and dates to be determined). Please note that an apprentice cannot be self-employed.

  • To be eligible for tuition fee funding through the Apprenticeship Levy, candidates should not normally have already completed a qualification at the same level as the proposed apprenticeship, unless that qualification was gained in a completely unrelated discipline.

The trainee Advanced Clinical Practitioner will assess, diagnose, treat and make the decision to admit or discharge frailty patients safely ensuring appropriate advice, health education and referral to appropriate services or other members of the MDT under the supervision of the Acute Frail & Complex Care team.

About us

Our Acute Frailty Team was established in 2018 and currently has 4 ACPs and a Consultant Frailty Lead.

The aim of the Frailty Team is to complete comprehensive assessments on frail patients with the focus to discharge at the front door, reduce overall LoS, reduce the potential of harm and improve the quality of care for patients.

The trainee post provides the successful applicant the opportunity to expand their medical knowledge by working in other areas such as medical, surgical and orthopaedic wards and in the community to give an overall of understand of the ACP role in other settings.

The role will be supervised by the Frailty Team and is heavily supported by a Consultant Geriatrician.

The trainee post is AfC band 7 and will progress to band 8a upon successful qualification.

Job responsibilities

The post is 37.5 hours per week, with 15 hours dedicated to the ACP programme and studies. The Acute Frailty Team provide an 8am-6pm, 7 day service throughout A&E, medical assessment units and short stay wards.
  • Under supervision, undertake advanced physical assessment and history taking in order to reach diagnosis and management plan for healthcare problems and conditions.
  • Request/perform diagnostic interventions, treatments as required, and initiating appropriate management.
  • Critically analyse and synthesize complex assessment data for sound decision-making.
  • Evaluate responses to treatment and initiating changes as required.
  • Make appropriate use of the evidence base, clinical guidelines and policies at all times.
  • Initiate and coordinate the treatment and education for individuals and their families/carers for their health protection, health promotion, and disease prevention.
  • Ensure timely review of patients on the ward/department as requested by ward staff in the interests of improved patient care.
  • Exercise good clinical judgement appropriate to level of competence, experience and skills.
  • Improve patient outcomes, service delivery and quality of care by bench marking national standards and implementing into practice.
  • Acknowledge limitations in knowledge, referring to senior medical staff when scope of practice is exceeded.
  • Make referrals of patients to and receive referrals from other healthcare providers to ensure continuous care.
  • The post holder will be responsible for their own learning requirements and completion of the course requirements.

Qualifications

Essential

  • GCSEs at grade C or above or grade 4 to 9 (or equivalent) in English and Mathematics
  • Current professional registration
  • First degree relevant to specialty
  • Willing to complete non-medical prescribing course


Desirable

  • Evidence of courses specific to Acute Frailty
  • Evidence of level 7 study or significant experience with own speciality


Experience

Essential

  • Minimum of 5 years post qualification experience in acute frailty/geriatrics or a closely related field


Desirable

  • Worked within an acute environment at a senior clinical level


Skills

Essential

  • Evidence of analytical and judgemental skills
  • Research skills


Desirable

  • Advanced clinical skills in patient assessment, diagnostics with division of treatment plan.


Knowledge

Essential

  • Knowledge of acute frailty and common presenting complaints relating to acute frailty
  • Awareness of national and local targets, NSF and policies in relation to acute frailty
