Job summary

You will be a skilled Registered Nurse with the appropriate attitude, skills andknowledge base. In addition, you will hold a Bachelors degree (or equivalent). You will deliver quality holistic care and treatment to the practice population. Whilst you are a dependent practitioner, you will be able to work independently and make independent decisions enabled by a collaborative and supportive working relationships with your clinical supervisor and large team of trained Advanced Practitioners. You will demonstrate safe and effective clinical decision-making and expert care, including assessment, diagnostic and management skills. As well as dealing with acute illness, you may be involved in the management of long-term conditions.

Main duties of the job

You will assist in the provision of patient care at our PCN Urgent Care Hub at theOxford Centre which manages acutely booked appointments by our 9 GP Practices. At times you may be required to travel and work from all 9 Practice sites in North West North Tyneside or establishments working in collaboration with the multidisciplinary team to deliver high quality patient-centred care.

The trainee Advanced Practitioner (tAP) will develop specialist knowledge and skills to provide healthcare autonomously to patients in primary care with a focus on acute minor illness through an established ACP training pathway.

Whilst in training the tAP will undertake a MSc in Advanced Clinical Practice hopefully as an apprentice, included in the curriculum are non-medical prescribing and work-based learning modules. As well as the academic pathway the tAP will follow the three-year GP and ANP-supported ACP training program mapped against established national and regional competencies.

The tAP will gain the skills under direct supervision to undertake clinical assessment, diagnosis and treatment of patients with undiagnosed and undifferentiated conditions. In addition to this the tAP will also be trained in the referral of patients to relevant specialities for any necessary inpatient or outpatient investigations.

The tAP will develop advanced skills in clinical decision making, critical thinking, and clinical management skills based upon evidence-based practice.

North West North Tyneside Primary Care Network is a collaborative workingarrangement of 9 GP Practices in North West North Tyneside serving a population of around 73,000 patients. It has around 50 staff covering acute care, medicines, first contact physio, social prescribing, mental health, research, training & development, learning disabilities, Nursing and HCAs. Over its 3 years of existence, it has grown to provide over 500 acute on the day appointments per week for patients alongside delivering all contractual services asked of it. We now feel we have reached a level of maturity to start to develop and grow the staff of the future and want to work with an outstanding and enthusiastic individual in this role to help us help them develop into a skilled Paediatric Advanced Practitioner over 3-4 years.
