The Primary Care Mental Health Liaison Service is looking for its remaining three staff to complete the roll out of this unique and ground breaking service to the citizens of Cardiff and Vale. Do you want to be part of a new development working across mental health and primary care services? If so, we want to hear from you.

As part of a Primary Care Sustainability Project, and the successful pilot of a new and innovative service in the East of Cardiff, we are now in a position, to roll out new opportunities to work in the Mental Health Clinical Board based in a primary care setting, in Cardiff and/or the Vale of Glamorgan.

Your role will consist of the provision practice-based Mental Health Professionals to support the work of General Practitioners, providing expert advice to both patient and primary care practitioner alike, assessment, referral on to appropriate services, signposting, follow-up reviews, and Mental Health First Aid. You will be instrumental in supporting patients much earlier in their pathway to improve patient experience and access to specialist support closer to patients home.

In addition, the UHB has introduced enhanced 3rd Sector support to provide Tier Zero and Guided Self Help (CCI) interventions within each locality, as a direct support to the new service.

The successful candidates must be able to demonstrate in-depth knowledge of Community Mental Health Practice. A good up to date knowledge of the delivery of psychological therapies and support is advantageous. We welcome applications from Registered Mental Health Practitioners from Nursing, Occupational Therapy or Social work.

You will work as part of Cluster Team, (providing services to a number of GP Practices) supervised by a Senior Band 7 Senior Liaison Worker. The service is headed up by a Service Lead who links the Mental Health and PCIC UHB boards to the Primary Care clusters in a co-produced service.

This new service forms part of the Psychology & Psychological Therapies Directorate, which is currently based at Global Link Building, between Penarth and Cardiff Bay. The successful candidates will be based in a practice(s) and appointments/bases will be guided by a project implementation plan. You can expect to receive excellent access to clinical supervision, peer support and further study to enhance career development.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
