NCH&C is proud to be the first standalone NHS community trust in the UK to achieve an 'Outstanding' rating from the Care Quality Commission (CQC). Our focus is on continually improving the quality of care we offer to local people and on improving access to that care, helping people to move seamlessly from one service to another. Praising NCH&C's "compassionate, inclusive and effective leadership at all levels", the CQC observed that our staff are well supported to make positive changes and innovations.

We welcome applications from people who share our values and can help us deliver outstanding care in our local community.

We have an exciting opportunity to recruit a Band 5 Community Nurse to join the Mid Norfolk and Breckland (Dereham/ Watton) Community Nursing and Therapy Team on 30 hours a week on a fixed term contract of 1 year. The Breckland Team is one of eight teams in the South Place within Norfolk Community Health and Care NHS Trust.

The post is based within the Dereham Community Nursing Team. This large, friendly and busy team requires a keen and motivated Nurse to provide assessment and quality interventions to the population of patients restricted to receiving care in their homes. You will be covering the geographical patches in and around the Dereham area.

You will be a registered nurse with skills in palliative / end of life care, wound care (including management of leg ulcers and pressure ulcers) for this vulnerable group of patients and a strong desire to further develop your community nursing skills and experience.

You will be required to work over a 7 day working week including weekends and be hard working, motivated, with excellent communication skills. You will be able to demonstrate knowledge and skills in holistic assessment, to demonstrate that you use your initiative and have a keen interest in working with this group of patients. You must hold a full and valid driving licence and have access to a vehicle.

You will support staff working at a band 3 and 4 level, along with students and apprentices.

This post is suitable for both newly qualified Nurses and experienced Nurses.

Apply now to join an organisation that has been awarded an 'Outstanding' rating by the Care Quality Commission (CQC), the highest possible rating and the first stand-alone NHS community trust in the country to be awarded the title.
