- Working as an active member of a multi-disciplinary team
- To undertake complex case arrangements for social care assessment and care planning in situations which involve risk, complexity and safety of the service user and others
- To provide more specialist care management practice skills and knowledge in regard to both processes and protocols within Children's and Adult Disability service user groups (including safeguarding) and to make independent decisions
- To ensure that in day to day practice the statutory duties of the organisation are being met
- To provide a safeguarding lead role within the team
- To represent the Department in inter-agency networks, working jointly with them in assessing, planning and implementing care to promote independence

- Ability to communicate effectively at all levels and proven ability to promote anti discriminatory practice
- Ability to exercise authority in representing the Department and making decisions independently
- Ability to build effective networks, both formal and informal which help promotes the independence of service users
- Competent in the full use of IT and positively promote its use within the team
- Demonstrable understanding of the importance of quality assurance systems and data collection
- Ability to promote professional development within the team, share skills and knowledge with others in ways that are empowering and ability to contribute to the training of others, both internally and externally
- Ability to organise own time effectively
- Possess a high level of self-awareness and understanding of how own behaviour impacts on others
