Currently, acute hospitals with Cambridgeshire and Peterborough are being prevented from performing their vital functions because their beds are filled by almost well people who are not able to return to their own homes - however with CPFT Intermediate Care they can.

Cambridgeshire and Peterborough has a vision for the future of health & social care in the County. Our joint plan to keep Cambridgeshire and Peterborough Fit for the Future has been developed by health and social care partners and sets out how we will redesign services to deliver the NHS England 5 Year Forward View.

As a result of the Covid-19 pandemic we are expanding our existing Intermediate Care Team to support our discharge to assess pathway. We are recruiting additional Specialist Nurses to support patients' transfer of care from our acute hospitals across Cambridgeshire and Peterborough. You will have a pivotal role in decision making as part of our system wide multidisciplinary single point of access triage, ensuring patients are on the most appropriate discharge pathway to meet their needs. You will also assess and manage patients in our commissioned health interim nursing home beds. You will do this as part of the wider Cambridge and Peterborough Health and Social Care system and work closely with therapy and integrated care workers in the Intermediate Care Service working with people in the health interim beds and in their own homes.

The vision of the service is to provide outstanding, patient focused, community based health care and therapy, optimising peoples potential to achieve their goals by supporting earlier discharge from hospital; early assessment for care needs and avoidance of hospital admission.
