We have an exciting opportunity for experienced and enthusiastic Band 5 nurses to join the North Place Supported Care Service as an Unplanned Care Nurse.

The Supported Care Service Unplanned care Nurse provides urgent, unplanned nursing support within 2, 4, or 24 hours of referral to patients in their home settings all over Norwich, including in residential homes and supported living complexes, in order to avoid unnecessary admissions to hospital. The post involves lone working and managing own caseload throughout the day. Good communication with the coordinating team at Supported Care and the ability to work as part of a multidisciplinary team is paramount.

The workload requires a good range of clinical skills to be applied in assessments and management of our patients with varied health problems including diabetes management, traumatic and post-operative wound care, pressure ulcer prevention and management, male, female and suprapubic catheter insertion, bowel care, palliative and end of life care, syringe driver initiation, stat doses and verification of death. Many of our patients have complex multi-pathologies and so we aim to provide holistic, patient centred assessments and deliver individualised care.

You will have a proven background in nursing and demonstrate an understanding of the roles of planned and unplanned community nursing. Alongside the clinical responsibilities, the Band 5 Unplanned Nurse will support colleagues and other team members through supervision. NCH&C is committed to the development of future health clinicians and there will be an expectation that the Band 5 Unplanned Nurse will support students and apprentices in the role of supervisor and as an assessor. You will also promote the NCH&C's Behaviour Framework.

North Place is working alongside its commissioners and local service providers as part of the Primary Care Network to develop community based services that support patients to remain at home and receive the services they require in an effective and co-ordinated way.
