We are looking for Registered Nurses to join our Rapid Response Team in West Kent. The role is diverse and the team work closely with other services to avoid admission to hospital, facilitate early discharge and provide unscheduled care to patients in their own homes.

You will carry out holistic assessments, develop integrated care packages, support junior staff and actively participate in a multi-professional team to ensure high quality care is provided to our patients.

The West Kent Rapid Response Team works from 2 bases, Coxheath and Tonbridge, cross site working is expected. The team operates 24 hours a day, 365 days per year.

You will be supervising Assistant Practitioners, Health Care Assistants and mentoring Student Nurses and Trainee Nurse Associates.

Caring for people in the community requires specialist knowledge and skills, we will develop you through our excellent educational opportunities that range from degrees to Apprenticeship- schemes.

Excellent communication skills both written and verbal are essential. You will be required to maintain a close working relationship with a range of other professionals within the Multi-Disciplinary Team. This is a community based post therefore full UK driving licence and access to a car is essential.

Part time hours / night shift only contract would be considered to support an individuals home/work life balance.

This means that we reserve the right to change the hours detailed in this advert to full time or part-time to meet the service requirements. Please note that the salary stated is based on full time working and will be pro rata for part time working.

Agenda for Change posts based within the Dartford, Gravesham & Swanley areas also receive 5% high cost area allowance on top of basic salary, subject to a minimum payment of £1,017 and a maximum of £1,762. Please note that this element does not apply to bank positions.
