As a Mental Health Case Manager, the post holder will work as part of a dynamic team in delivering an effective service supporting managers and staff across the North East and Yorkshire. The case management function supports a vital role in ensuring that safe and effective care pathways are in place for all service users of specialised mental health care settings within the NHS and Independent sectors.

The post holder will be experienced in working within a busy health care setting and be skilled in quality and performance management. The post holder will have a working knowledge of patient treatment pathways and monitoring progress of patients within inpatient settings.

You will be expected to take a leading role in projects relating to quality assurance and will build and maintain strong relationships with providers, other commissioners and support agencies as appropriate.

The role supports the business in driving transformation as well as value for money in planning, commissioning and service. The role is designed to build a combination of subject matter expertise and technical skills to develop a strong service delivery.

As part of the Local Provider Collaboratives, some posts will transition to providers from October 2021. This will be discussed further as part of the interview process.
