We have an exciting opportunity for an experienced and enthusiastic nurse to join the Central Norwich Neighbourhood Community Nursing Team for a period of nine months from September 2021, in a 22.5 hours per week fixed term temporary post or secondment. This post will be covering maternity leave.

It presents interested nurses an opportunity to develop their senior nursing skills and give them the tools and experiences to support further career development in community nursing.

The Community Nursing Service treats patients predominantly in their home settings, including residential homes and supported living complexes. We also provide some services in a clinic setting including our leg ulcer clinic and catheter clinic.

The workload requires a good range of clinical skills to be applied in managing our patients with varied health problems including diabetes management, traumatic and post-operative wound care, pressure ulcer prevention and management, catheter management, bowel care, palliative and end of life care. Many of our patients will have complex multi-pathologies and so we aim to provide holistic, patient centred assessments and deliver individualized care.

You will have a proven background in nursing and a good understanding of community nursing. The post holder will support the Clinical Operations Manager in delivering a quality service to patients through the review and management of complex patients, supporting roster management and reviewing and implementing learning from incidents. You will also live, breathe and promote the NCH&C's Behaviour Framework. This Band 6 post also involves supporting team members through structured supervision and appraisal, as well as through more ad hoc clinical support. The Trust is committed to the development of nursing staff for the future through direct entry students and apprenticeships and so mentoring skills are essential.

Norwich locality is working alongside its commissioners and local service providers as part of the Primary Care Network to develop community based services that support patients to remain at home and receive the services they require in an effective and co-ordinated way.
