We are looking for Registered Nurses to join our Urgent Care Service based across West Kent in the role of a Team Leader.

Our Urgent Care Service provides a two-hour response to patients in crisis at home. Our aim is to prevent unnecessary hospital admission and facilitate early discharge from the acute hospitals. We also provide unscheduled care out-of-hours for patients who may be at the end of life, or experiencing an urgent need for healthcare. The Ten Year Plan for the NHS emphasises the importance of community-based care and a collaborative approach to the delivery of health and social care, meaning there has never been a better time to join us.

You will work alongside registered nurses and therapists, doctors, advanced clinical practitioners, other health care assistants and administrators, providing a dynamic, personalised and responsive service.

You will carry out detailed holistic assessments, developing personalised plans of care in collaboration with patients and their carers. Our patients are complex, and require proactive risk management, in order to be maintained safely at home. You will be accountable for maintaining clinical oversight, and ensuring all actions are completed appropriately. You will need a wide clinical skills base to provide hospital-level care at home. You will be a role model for junior staff supporting their development, as well as supervising student nurses and trainee nurse associates. You will actively participate in a multi-professional team to ensure high quality care is provided to our patients.

Your organisational skills will be essential in the planning and allocation of work and the management of the caseload, to ensure timely patient interventions and flow through the caseload. Flexibility is a key skill, as the acute nature of the patients requires frequent triage and prioritisation. You will be responsible for identifying pathways of care for each patient to ensure timely admission and discharge from the service.

You will be accountable for the safe and effective delivery of care to a caseload of patients, and for the management of a team of staff working with you. This includes one-to-one support, absence management and promotion of wellbeing. Part of the role involves investigating incidents and complaints, embedding changes in practice and leading on team innovations, developing the service.

The Urgent Care Service operates across 24 hours, with early, late and night shifts. As a Registered Nurse, you will be working across all shift patterns, including weekends and Bank Holidays.

Caring for people in the community requires specialist knowledge and skills, we can develop you through our excellent educational opportunities that range from degrees to apprenticeship schemes.

This is a community-based post therefore full UK driving licence and access to a car is essential, subject to the provisions of the Equality Act (2010).

Part time hours/night shift only contract would be considered to support an individual's work/life balance.

This means that we reserve the right to change the hours detailed in this advert to full time or part-time to meet the service requirements. Please note that the salary stated is based on full time working and will be pro rata for part time working.

Agenda for Change posts based within the Dartford, Gravesham & Swanley areas also receive 5% high cost area allowance on top of basic salary, subject to a minimum payment of £1,017 and a maximum of £1,762. Please note that this element does not apply to bank positions.
