Midlands Partnership NHS Foundation Trust (MPFT) was formed in June 2018 when South Staffordshire and Shropshire Healthcare NHS Foundation Trust (SSSFT) acquired Staffordshire and Stoke-on-Trent Partnership Trust (SSOTP). For further information please click on the Trust name above.

The Trust works in partnership with the NHS programme 'Step into Health', supporting ex-service personnel into a career in the NHS.

There has never been a better time to join the Trust and help shape its future; take a look at our Benefits Leaflet to see what else we can offer you.

An exciting opportunity has arisen for a Band 5 Community Staff Nurse to join the Out of Hours community nursing service. We have full time and part time hours available with shift patterns that include evening and nights spread over a 7-day week service covering the hours of 18:00 - 08:15. We are looking for an experienced nurse who is passionate about providing care in the community.

The Out of Hours team is a continuation of care from the day teams, thus providing a 24 hour nursing service in the community to service users in their own homes or residential homes. Staff are expected to work autonomously and must be prepared to travel and organise time accordingly. We have technology and lone working procedures in place to support staff during visits.

You must also be able to care for people who live in the community, who are at risk of deteriorating health resulting in a reduced quality of life, with an emphasis on avoiding hospital admission or unnecessarily long lengths of stay. We are looking for a dynamic, enthusiastic and motivated individual to join our Integrated Locality Care Team. There will be opportunities for personal development.

You should have excellent communication and interpersonal skills and the ability to organise and prioritise your workload without direct supervision. You should be committed to delivering a high quality service through an integrated and holistic approach to patient care.
