Primary Care Links - Project Lead, Social Prescribing

22.5 hrs

Salary - £27,730 (pro-rata)

Fixed Term contract to 31 December 2025

York CVS has been managing a Social Prescribing services since 2015, supporting people who have been referred via their GP/practice nurse or Primary Care worker. Social Prescribing empowers people to take responsibility for their own health and wellbeing by linking them to non-medical support from within the community. We take a holistic approach and provide person centred support to individuals to improve their health outcomes.

The Link Worker programme is a national initiative from NHS England and NHS Improvement. This post is a collaboration between four of the Primary Care Networks (PCN's) in York and York CVS.

The Project Lead will work in collaboration with each PCN to deliver a coordinated and high-quality Primary Care Link Worker (PCLW) service. They will lead, supervise and support the team of PCLWs to deliver a range of wellbeing and social prescribing interventions.

The successful candidate will have significant experience of working in the healthcare, social care or voluntary sector, with vulnerable adults. They should have excellent communication skills and good organisational skills with a minimum of one year management experience in a similar setting.

Please note that this role may require occasional evening and weekend working.
