An excellent opportunity has arisen for a community matron to support the enhanced health in care home project across Tees Community Services.

There will be a dedicated clinical team of two band 7 community matrons appointed to support the Enhanced Health in Care Homes (EHiCH) Direct Enhanced Service for 9 months. The dedicated team will be separate from the current community matron team and will be entirely focused on supporting and developing the EHiCH MDT and Home Round models, and active MDT based case management (where appropriate) across the 6 PCNs.

The community matrons will work collaboratively with the PCNs , GP practices, TEWV EHiCH Leads, the Single Point of Access and other partners to support the move to a more standardised model of MDTs developed with partners, across PCNs and care homes. Using senior clinical support will provide credibility with clinicians to ensure MDT's are coordinated effectively with all partners and support the collaborative approach to delivery of MDTs as part of the EHiCH Framework.

The Matrons will also support the PCNs in the review of care home residents who are new to the home or discharged from hospital to ensure residents to have up to date personalised health care plans which have been developed through collaboration, including advanced health care plans This will ensure all residents, including those who are stable and do not require intervention through the weekly home round or MDTs, receive the EHiCH offer. It will also support on-going monitoring and evaluation of the EHiCH delivery model in South Tees.

Leadership and Improvement Training
South Tees Hospitals NHS Foundation Trust offers leadership and improvement training to all new staff to the Trust; this training programme has been designed to support our leaders in developing their understanding of leadership and management skills. You will be expected to attend the leadership development programme, and the Quality Improvement Programme that the Trust delivers. This programme aims to:
  • Explore leadership within the NHS
  • Promote Trust Values and Behaviours
  • Develop your Leadership effectiveness and skills
  • Practitioner level in quality improvement, equipping you with the skills to champion, lead and complete quality improvement within your role

After you complete this four and a half days of training you will be able to explore further leadership and improvement training opportunities, we offer further in-house courses dependent upon your role and bitesize programmes and leadership apprenticeships at level 3,5 and 7.
