As a Care Coordination Team Leaderwithin Virgin Care BaNESyou'll join a team who are focused on clinical excellence and who were dedicated to support patientsduring the COVID Pandemic, continuing to keep the critical service running.Based at our Care Coordination Centre in Peasedown, B&NES, you will lead our non-clinical team ofCare Coordinators that provide exceptional, consistent levels of service to both our patients and our clinical teams to achieve high levels of satisfaction via the telephone and IT systems. You will join our growing developing team who are focused on achieving best results.

Your days will be varied and fast pacedand include working closely with Duty Clinicians, Nurses, Wellbeing, Social Care and Integrated Care Teams. You partner with our existing Team Leader, leading the team that is our first point of contact; booking appointment, managing referrals, signposting and providing information/ advice for a range of different community healthcare services and managing multidisciplinary meetings.

You will have a car to travel to Virgin Care BaNES locality offices supporting your staff to coordinate tasks and activities.
