Essential Qualifications:
  • Care Certificate - or completion of the Care Certificate within 12 weeks of start date.
  • NVQ L3 in H&SC, Diploma L3 in H&SC or apprenticeship framework L3 in H&S care
  • Foundation Degree in Health/social/mental health L5

Full time (37.5 hours) or part time hours considered.

Can be based at Gt Yarmouth or Lowestoft.

Role Summary:

The Central requirement is that the post holder will have a generic approach to the care of the service user, utilising a wide range of traditional discipline boundaries (as professional registration allows). These tasks will be determined by the needs of the individual e.g. supervising medication, shopping, social integration, maintaining and improving their home environment.

To work alongside service users, facilitating their development of daily living/social skills.

To maximise their potential and opportunities to participate in work/ training / education / social activities.

Duties And Responsibilities:
  • Undertake the Care Certificate within the first 12 weeks of employment - or have an existing qualification agreed as meeting partially - or wholly the standards of the Care Certificate.
  • Form effective, supportive working relationships with service users and their carers.
  • Provide practical help and emotional support to service users in line with their care plan, as formulated with in CPA care planning and directed by their care co-ordinator, including telephone support.
  • Contribute to the care planning and review process.
  • Contribute to the assessment of service users' needs directed by the care coordinator/other registered member of the multi-disciplinary team.
  • Keep accurate and timely records of work and observations of service users wellbeing/progress including the updating of electronic records.
  • Develop effective / supportive working relationships with other members of the multidisciplinary team.
  • Be alert to any issues that may be arising in relation to the service users and to report these rapidly and clearly to the care co-ordinators / other members of the multidisciplinary team.
  • Will have a basic understanding of medications used and their side effects
  • Following direction by a registered member, may deliver medication dispensed from hospital pharmacy to a patients' home. The post holder must comply with the procedure for transportation of medicines as per Trust Medicine code. Must have knowledge of the patients on going care plan and the relevant risk assessment and management plan.
  • At the direction of a registered member may observe patients self-medicating in their own home to monitor compliance.
  • The post holder must ensure correct record keeping procedures are adhered to and report their observations to a Registered Practitioner. These observations must include patients' mental and physical health as well as any side effects observed. At no time must the post holder administer medication.
  • Observe strict confidentially and maintain high standards at all times.
  • Treat service users and carers with respect, promoting equal opportunities whilst endeavouring to develop an understanding of their problems/needs and perspective on life. To pay close attention to cultural and spiritual needs of ethnic minority groups.
  • To use own judgement and initiate action where appropriate training needs of self and the team and convey these to the manager via the appraisals process.
  • To be responsible for ensuring own technical competence, and to be aware of the operation, care and maintenance of any equipment in your use.
  • Identifying and, where possible, reducing family and interpersonal stresses on the client by education, problem solving, relationship skills, support and sign posting got helpful resources.
  • Advocacy, for and on behalf of the clients to ensure full access to community resources, amenities and state benefits.
  • Transporting the client when needed in the care plan to attend important appointments eg DSS, social services, hospital etc.
  • Liaison with social services, police, prison service, courts etc.
  • Actively seeking out, developing and sustaining close links with community agencies in order to facilitate the delivery of the most appropriate and best possible care for the individual client.
  • Liaison with GP surgeries and with home care, residential placements etc
  • Assist in the induction of new staff.
  • To participate in continued professional development, attending relevant training courses to promote evidenced based practice.
  • To utilise through knowledge and skills acquired through training and share with colleagues.
  • To participate in an annual personal development plan and appraisal.
  • May be required to deal with clients' finances.
  • Follow policies, may comment on proposals for change.
  • Organise own workload, services to support clients.

  • Experience working with people with severe and enduring mental health problems.
  • Dealing with challenging behaviour.
  • Ability to work under pressure.
  • Community based experience.
  • Demonstrate flexible approach to care.
  • Demonstrate an ability to work independently yet function as part of a team
  • Be respectful, empathic, empowering, good communication skills, problem solving skills and flexible approach.
  • Satisfactory verbal and written English language skills

Knowledge:
  • Overview/Knowledge of mental health and related issues.
  • Knowledge of the principles of recovery focused care.
  • Understanding of social integration.
  • Principles of person centred planning.
  • An understanding of social integration
