We are looking for Band 4 Senior Rehabilitation Support Workers to join us in our expanding and award winning services, which provide healthcare at home and prevent admission into our hospitals.

You will have access to on-going support within our services.

    Our services:

    Intermediate Care Lambeth provide healthcare at home to adults aged 18 years and older who live in the boroughs of Lambeth, South London. The people that we support have a range of diagnoses and clinical presentations, including falls, orthopaedic, respiratory, frailty and neurological conditions. We:

    1. Prevent unnecessary hospital admissions
    2. Facilitate the safe discharge of people from healthcare in hospital to healthcare at home
    3. Provide intensive, short term multi- disciplinary rehabilitation and care support to promote functional independence.

    Healthcare in the home is rapidly increasing and enables people to be treated at home where they are more comfortable. This helps us to provide a more holistic, well-rounded and bespoke healthcare service to the people we support.

    The Trust offers our clinical support staff extensive training from a wide range of areas including:

    • Opportunity to complete a Foundation Degree in Healthcare
    • Apprenticeship schemes
    • Comprehensive competency programmes
    • Local induction to the department and speciality
    • An annual performance development review with a 6-monthly review
    • Specialised in-service training programmes
    • Peer support
    • Completion of a Care Certificate for Support Staff
    • Provide you with a mentor


Important note: The role requires 7 day working and shift cover (early 07.30-15.30 & late 14.00-22.00) on a rota basis.
