An Excellent opportunity has arisen within Middlesbrough, Redcar & Cleveland Community Matron Team. We are looking for skilled and outstanding senior clinicians to join our Community Matron/Practitioner service within the Growing the Friarage and Community Collaborative.

If you possess innovation, dynamism, drive and are passionate about patient focused care, this could be the challenge you are looking for.

You will have significant post-registration experience, be a non-medical prescriber and have advanced clinical skills to help you effectively manage a caseload of patients with highly complex needs. You will utilise your case management approach which will have a direct and positive impact on the person's ability to cope with their long term conditions with the aim to reduce unnecessary hospital admissions.

In the role of the Community Matron/Practitioner you will be a key member of the upcoming primary care networks and our unplanned service. You will possess excellent communication and interpersonal skills and be able to demonstrate the ability to work across a diverse range of clients with complex needs.

You will also participate in the unplanned Rapid Response Service providing a rapid assessment of nursing care and treatment for patients in their own home to prevent unnecessary admissions and/or facilitate timely discharge from hospital.

You will also be involved with the care of patients across the area that reside in designated Intermediate Care beds and will be influential in the expansion and integration of this service.

The successful candidate(s) will join a team of Community Matrons/Practitioners with a wealth of clinical knowledge and experience who will support and encourage you in your new post.

Consideration will be given to candidates who do not possess non-medical prescribing qualification for a band 6 training post to become band 7 once this qualification is achieved, however Physical assessment skills is essential.

Experience of Community nursing, working within an emergency assessment unit or rehabilitation would be an advantage and we welcome informal visits to the areas.

South Tees Hospitals NHS Foundation Trust offers leadership and improvement training to all new staff to the Trust; this training programme has been designed to support our leaders in developing their understanding of leadership and management skills. You will be expected to attend the leadership development programme, and the Quality Improvement Programme that the Trust delivers. This programme aims to:
  • Explore leadership within the NHS
  • Promote Trust Values and Behaviours
  • Develop your Leadership effectiveness and skills
  • Practitioner level in quality improvement, equipping you with the skills to champion, lead and complete quality improvement within your role

After you complete this four and a half days of training you will be able to explore further leadership and improvement training opportunities, we offer further in-house courses dependent upon your role and bitesize programmes and leadership apprenticeships at level 3,5 and 7.
