Northamptonshire's Post Covid Assessment Service is expanding due to increased demand for services and additional national funding to support this important work. We have vacancies for enthusiastic and empathic care co-ordinators who will work with the established therapy team to provide patient centred care and therapy interventions to people identified as suffering from Post Covid syndrome. The role will include home visits, liaison with a range of health professionals and monitoring rehabilitation plans in the community. The Care co-ordinators will be responsible for maintaining accurate records, organising MDT reviews for complex patients and co-ordinating actions with partners across Northamptonshire.

A background as an HCA or therapy assistant would be useful experience to bring to this role, as the knowledge around Post Covid syndrome is emerging, it is most important that you have enthusiasm for learning and ability to adapt to new circumstances and work processes as they emerge.

You will be joining a committed and professional team who are patient centred focused on enabling patients to manage their rehabilitation journey. You should be able to travel to appointments across the county and work on your own initiative with support from the wider team, which includes colleagues from other health & care providers in Northamptonshire.

We are hoping to recruit staff who can work predominantly in Northampton and the south of the county , or be willing to travel to these areas for work.
