We have opportunities within our new 'Hospital at Home' service for Assistant Practitioners who are looking for a new and exciting clinical challenge .

The Hospital at Home Service is the next stage of development in our two-tier approach to identifying and managing the deteriorating patient. The Assistant Practitioner will work under the direction of an Advanced Clinical Practitioner to deliver care interventions and evaluate their effect as a means of preventing hospital admission for patients who are acutely unwell in the community. They will work closely with other clinical services and the operational team to provide for a short period of intensive support for a small cohort of patients with acute health care needs.

To help develop and deliver this exciting new initiative we are looking for confident skilled Assistant Practitioners with experience in a primary, urgent or emergency care setting; who have an understanding of the philosophy and principles of health and social care and the organisation, function and physiology of the human body; and are reliable, dependable and flexible. We welcome applications from Assistant Practitioners who have completed on-the-job training or apprenticeship and obtained a healthcare qualification, such as the NCFE Council for Awards in Care Health and Education (CACHE) Level 3 Diploma in Healthcare Support, Level 5 Diploma of Higher Education, a Foundation Degree and the Level 5 Diploma for Assistant Practitioners in Healthcare. Further training and support will be provided to successful applicants to ensure they possess the necessary knowledge and skills to deliver a safe and effective service.

The Hospital @ Home service will operate between the hours of 08:00 and 22:00 daily and applications for full and part-time roles will be considered.

We can make reasonable adjustments and offer support and advice in a variety of ways on request.

Alongside an attractive salary which is equivalent to AfC Band 4(depending on experience and qualifications) we also offer NHS pension scheme, free parking, approachable supportive management teams and expert support and professional development from our Nurse Consultant and Clinical Service Leads.

We are a supportive, dynamic and proactive clinical team continually developing and expanding our services and exploring new and innovative ways of working to ensure we continue to deliver high quality care to our patients. We offer CPD and provide peer support and supervision along with regular training sessions.
