Requirements for this role:

Experienced in Older Adult Mental Health, preferably having experience in liaison psychiatry.

JOB SUMMARY:

The post holder will be joining a well-established team working with older adults (65+) admitted to Royal Berkshire Hospital (working hours Monday-Friday, 9am-5pm, full-time, permanent). The team supports patients in the Emergency Department as well as all medical wards across the hospital. Patients are primarily presenting with mental health or cognitive difficulties in the context of declining physical health and/or neurological or neurodegenerative conditions. The nature of the work is varied and dynamic and there are many opportunities to develop close working relationships with specialist departments, including diabetes, heart failure, oncology, stroke, care of the elderly and palliative medicine.

The post is placed within a supportive multidisciplinary team of mental health practitioners, including psychiatric nurses, psychologists, social workers, and consultant psychiatrists. The older adult service (65+) is a small team, working alongside the adult (18-65) psychological medicine team as part of the Psychological Medicine Service within Royal Berkshire Hospital. The overall service is an established CORE 24 service as part of NHS 10-year plan and is currently accredited with the Royal College of Psychiatrists.

The post would be best suited to a senior practitioner with either community or inpatient older adults experience, who is passionate about providing mental health care for older people and brings with them a strong interest in innovation and managing mental health in the context of physical health difficulties and/or declining cognition.

The role would best suit a practitioner from a mental health nursing, or a therapeutic background. They will be employed by Berkshire Healthcare NHS Foundation Trust as the provider of mental health services in Berkshire and hold an honorary contract with Royal Berkshire NHS Foundation Trust.

RESPONSIBILITIES: (Included but not limited to)
  • To be responsible for the mental health care of patients admitted to the Royal Berkshire Hospital.
  • To undertake comprehensive initial assessments of the mental health needs of older people who are currently receiving treatment for physical health problems within a general hospital setting considering their physical, psychological, social, and spiritual needs.
  • To have expert knowledge of a variety of assessment and screening tools e.g., MMSE, MOCA, GDS, HADS etc.
  • To plan, facilitate, deliver, and evaluate specialist person-centred care and treatment for older people with mental health problems in the context of their physical health and current admission, and where appropriate include the views of their family and carers.
  • To participate in joint assessment and care planning for older people with mental health needs within a general hospital environment.
  • To provide short term mental health interventions to older people referred to Psychological Medicine.
  • To provide counselling and emotional support to patients referred.
  • To communicate with, educate and support carers as required.
  • To offer consultation and highly specialist advice to colleagues from the general hospital multi-disciplinary team, regarding appropriate treatment and management of older people who present with mental health problems in the context of declining physical health.
