JOB SUMMARY:

To support the Ward Manager and junior sisters/deputies:

Act as a role model demonstrating clinical leadership, managing, and delivering high standards of individual patient care.

Work as a member of a multidisciplinary team using a combination of skills and knowledge to identify health and nursing needs and develop strategies to promote healthcare and comfort whilst recognising cultural beliefs and values of the individual,

Prevent patients having unnecessary acute hospital admission and/or facilitate timely discharge.

Promote staff development.

Ensure robust and effective infection control practices amongst all staff.

Maintain and promote the reputation of BECHS by displaying attitudes and behaviours commensurate with the Trusts vision statement and ensuring other members of staff understand their personal responsibilities to support this aim.

KEY responsibilities:
  • Demonstrate excellent personal communication skills, ensuring two-way communication channels with individuals and groups are established in order to maintain collaborative team working.
  • Discuss with patients and their families/significant others, when appropriate, their goals and gain consent and active participation in the proposed care.
  • Act as part of the senior cover rota and respond to queries and interruptions through the day.
  • Maintain comprehensive and accurate patient records in line with legal and Trust requirements.
  • Communicate holistic assessment and care plan results to the appropriate member(s) of the multi-disciplinary (MDT) team including in writing as required.
  • Communicate and document any changes in care plan or treatment to patients and the appropriate MDT member.
  • Participate in relevant ward rounds, meetings and case conferences and produce written reports and patient letters as appropriate ensuring all relevant information is communicated and updated, to ensure effective and timely discharge of patients.
  • Contribute to the assessment of the mental capacity of patients and gain valid consent, having the ability to work within a legal framework with those who lack capacity to consent to treatment.
  • Ensure the identification and escalation of inline BECHS Deprivation of Liberty policy any care regimes which may be or result in, a deprivation of liberty of an inpatient (DoLS 2007 supplementary code MCA 2005)
