Specialist services lead on best practice patient centred care in the management of each of their specialist areas e.g. diabetes, respiratory. For some people, coronavirus (COVID-19) can cause symptoms that last weeks or months after the infection has gone. This is sometimes called post-COVID-19 syndrome or "Long COVID". Sirona has developed a therapyservice for people with persisting Long Covid symptoms which are significantly impacting their day to day life.

We are delighted to be expanding our team with this new Care Coordination role. We are looking for an organised and motivated individuals to join the Specialist Services team to support the new model of care being developed to support people with Long Covid and work with teams to enable seamless delivery.

You will need to be self-motivated, a good communicator, excellent prioritisation skills to manage multiple priorities in a flexible way. We will support you with training and development plan to enable you to complete the role as required

Working hours of the service for specialist advice are from 8am - 6.30pm Monday to Friday (excluding bank holidays) and this role is predominately based at New Friends Hall, Stapleton although there is opportunities to work from home. There is also a requirement to travel as part of the role between sites occasionally so access to a car is preferable

KEY RESPONSIBILITIES:
  • The role provides prompt, efficient co-ordination support to the Long COVID multi-disciplinary team (MDT) ensuring prompt response to referrals and co-ordinates interventions by team members and ensures that referrals are made promptly to other services.
  • This role will involve taking responsibility for an identified and allocated caseload following screening assessment by a registered practitioner.
  • The role will work independently in the community setting providing the appropriate level of therapy for each individual client within set parameters of their therapy programme using own clinical judgement and risk assessments.
  • Work with relevant MDT professionals to support the assessment and care provision processes including tracking progress.
  • The role will maintain the information systems used by the team in order to collate and co-ordinate performance data and produce reports as required for the team and relevant managers.
