We currently have a vacancy within our North Transfer of Care Team working within Hinchingbrooke, Peterborough City Hospital and Queen Elizabeth Hospitals and opportunities to work within the community under discharge to assess (D2A) operational model.

You will be imaginative and dynamic and make a positive contribution to delivering good outcomes for people who have care and support needs to leave hospital and post discharge. As well as being able to work within an integrated setting, you'll be a person-centred practitioner with excellent communication skills..

Your role will include undertaking Care Act Assessments, Mental Capacity Act Assessments and Safeguarding work. Applicants must be experienced social workers and currently registered with the HCPC and have knowledge of the legislative framework related to adults.

IMPORTANT: This role will require you to enter the premises of a CQC-registered care home as part of your employment. Regulations which come into force from 11th November will require employees over 18 who enter CQC-registered care homes in Englandto have completed a full course of authorised COVID-19 vaccine doses at least 2 weeks before 11thNovember, unless they have a medical exemption. As part of your pre-employment checks you will need to provide evidence of your vaccinations or medical exemption. Your employment will not commence until this evidence is received.
