Midlands Partnership NHS Foundation Trust (MPFT) was formed in June 2018 when South Staffordshire and Shropshire Healthcare NHS Foundation Trust (SSSFT) acquired Staffordshire and Stoke-on-Trent Partnership Trust (SSOTP).

We have specialist mother and baby, eating disorder and medium secure forensic mental health services and contracts across the country for IAPT, addiction and some health services in prisons as well as hosting a network for inpatient mental health care for serving military personnel.

The Community Intervention Service provides care to patients in their own homes and is pivotal in the prevention agenda. This includes supporting the care of frail older people with long term conditions.

As a Community Staff Nurse within the Community Intervention Serviceyou will deliver a high quality service, working closely with GP's and multi-disciplinary teams in addressing a range of care issues across the spectrum of health conditions, and preventing avoidable hospital admissions as well as supporting to expediate discharge from hospital.

We are an innovative service covering GP practices in theStafford and widerlocalities and we are seeking enthusiastic and highly motivated nurses.

You will work independently in a community setting, participating in the ongoing assessment, planning, implementation and evaluation of programmes of care, adhering to policies and procedures.

You must be a qualified nursing practitioner with current NMC registration. Previous experience of working in the community is not an essential requirement, however, you do need to be able to demonstrate a commitment to delivering high quality targeted personalised care.

We willwelcome applications from student nurses who are graduating as part of the March 2022 cohort

Post holders will be required to work flexibly on a rota system across a 7 day working week.
