South West Yorkshire Partnership NHS Foundation Trust

Opportunities for Band 5 and Band 6 Community Mental Health Nurses

North Kirklees Enhanced Pathway, Adult Mental Health Community Services

The Enhanced Pathway provides evidence-based and therapeutic interventions for people with complex mental health needs. Our teams work with service users and their families and carers to enable them to live well in their communities.

We are looking for individuals who are flexible and creative in response to organisational and service user needs, and who are committed to developing high-quality, strengths-based and recovery-focused packages of care and support.

You will be responsible for the co-ordination of care delivery within our integrated team of health and social care staff, which will involve you undertaking holistic assessments, care planning and delivering treatment interventions.

We want to make sure our staff have the right knowledge and skills to deliver safe, modern and transformational interventions, so that people with mental health difficulties are treated with dignity and respect and receive the care and support they deserve.

We want you to feel safe and supported in your practice and will help you to grow and develop, with a strong emphasis on your professional wellbeing and career satisfaction. You need to be open to ongoing learning and development, to continuously refresh and augment your knowledge and skills.

We are inviting applications from both existing Band 5 and Band 6 Community Mental Health Nurses. For those who are recruited into a Band 5 position, we will provide you with the support to progress into a Band 6 post within the team, through a career and competency framework developed within our Trust.

Progression from Band 5 to Band 6 will not happen automatically or be awarded solely on a time served basis. It will be determined based on practice evidence that the individual is working at a Band 6 standard, according to clearly defined core competencies in the development framework.

If you would like further information about these posts, please contact Ryan Groves (Team Manager) on 01924 316855 for an informal discussion.

The Trust is an Equal Opportunities employer and is also committed to a healthy work life balance. All applicants will be assessed fairly and on merit.

Car driver status is essential to this post.

As a flexible employer encouraging a healthy work/life balance, we will consider Part-Time and flexible working opportunities for all our positions.

Our application process will test your attitudes and behaviours, as well as your knowledge and skills. So that we know we'd be a good match for each other. If you're successful you'll then have a 12 months values based induction.

You'll also have access to a wide range of wellbeing support because our wellbeing at work matters to #allofus. There's a great range of support for us all including a workplace physio, staff retreats, health checks, counselling, mindfulness and a variety of sports groups. You can take part in our award-winning Creative Minds activities as well as access courses at our innovative recovery colleges.

We welcome applicants with lived experience of providing unpaid care and/or support to a family member or friend with a disability, health condition, frailty, mental health problem, addiction or other health and care needs.

We know there's a wealth of talent among people who have a disability as well as those who have experience of mental or physical health problems. So, if you need any support with your application just give us a call on 01226 644072.

A couple of things to note - if you're shortlisted for interview we'll contact you via your NHS jobs account, so please check this regularly. Please provide email addresses wherever possible for your references.
