Gloucestershire Health and Care NHS Foundation Trust was formed in October 2019, following the merger of 2gether NHS Foundation Trust and Gloucestershire Care Services NHS Trust. The two Trusts joined together to provide integrated mental health, physical health and learning disability services to communities.

Hours per week - 37.5 hours per week. Shift work either 4 days on 4 off or if part time 2 days on 6 days off

If so this may be the role for you. The Reablement and Home First Team provide vital short-term services to people by helping them to regain or attain as much independence as they can in their own homes. The therapy led team works to: resettle people after a hospital stay and with people to identify and work on goals that will help them to live well at home.

Rehabilitation Assistants work as part of a close team undertaking activities according to their individual goal plans, encouraging them to do as much as possible for themselves, to live well and safely in their own homes.

Rehabilitation Assistants do this by motivating and inspiring people to re-learn daily skills and enhance their quality of life.You will provide practical and 'hands-on' personal care as well as encouraging people and working with them to do the things that are important to them.

You will need to be able to drive around the locality to work with people in their homes.Team working and communication skills are essential. You willupdate written notes and electronic records.You will be fully trained to assist people to meet their needs, which can range from motivational support to personal care.

The service runs between 7am and 10pm, 365 days a year. There are pay enhancements for unsociable hours, as stipulated by Agenda for Change. For more information please see the Additional Information link to the shift patterns and search for NHS Unsocial Hours Payments for more information.

Split Shift: 07:00 - 12:36 / 16:54 - 22:00

Early: 07:00 - 18:06 (-30 minute unpaid break)

Late: 10:54 - 22:00 (-30 minute unpaid break)

AM: 07:00 - 12:00 (4 on 4 off or 2 on 6 off)

PM: 18:00 - 22:00 (4 on 4 off or 2 on 6 off)

Successful applicants will have the opportunity to discuss the prospect of undertaking a Level 3 Healthcare Support Worker Apprenticeship alongside their substantive post, this apprenticeship would include the Level 3 Diploma in Clinical Healthcare.

As a Health Care Assistant/ Support Worker you will be responsible for participating and assisting in the delivery of care / services to patients and service users as part of a Mental Health and Social Care Health Service. This post requires the post holder to be able to undertake all statutory and mandatory training and post holders must be able to undertake physical interventions. Fitness for physical intervention will be assessed by Occupational Health.
