New Year, New Job? We are looking for a range of Rehabilitation Support Workers and Healthcare Assistants to support our Community!

As a Rehab Support Worker you will be supporting registered healthcare professionals to deliver rehabilitation programmes and assist with care and support of patients referred to the service. You will also provide non-clinical support to the Community Therapy Team.

We have opportunities in our:
  • BIC Integrated Network Team(INT), based at East Trees Health Centre - Providing nursing and therapy services to the local areas ofMontpelier, Eastville, Easton, Barton Hill, Hotwells and Bristol City Centre.
  • FOSS Integrated Network Team(INT), based inSt. George (BS5) - Providing nursing and therapy services to the local areas ofSt. George, Hanham,Fishponds, Mangotsfield and Staple Hill in the East locality of Bristol
  • FABB Integrated Network Team(INT), based at Fishponds Health Centre - Providing nursing and therapy services to the local areas ofFishponds, Downend, Mangotsfield and Staple Hill in the East locality of Bristol

You will provide assistance to clinical staff in the assessment and treatment of patients in the rehabilitative process and follow a prescribed care plan to support service users at home including support with activities of daily living tasks and provision of aids to support independent living. This will include:
  • Practice, reinforce and progress activities of daily living e.g. personal care, dressing, meal preparation etc, as appropriate
  • To participate and support in promoting healthy and independent living practices to patients, families and carers.
  • To advise the Team Co-ordinator/lead therapist when patients support needs are no longer as per their care plan- e.g extra support is needed to help a service user achieve their goal or remain in their own homes
  • To work collaboratively with other organisations across different sectors of health and social care.
  • To support in the monitoring, ordering and receipt of equipment into the team peripheral store.
  • To carry out general administrative and non-clinical work to contribute to the functioning of the team such as photocopying, filing, liaison and telephone calls.
  • To document electronic case notes, which meet Sirona's Standards for documentation.

We are looking for experienced workers within the healthcare industry to support our growing number of service users in their journey to independence through the delivery of person centred rehabilitation programmes. Successful candidates will have to be comfortable working on their own in a community setting, have excellent verbal communication skills and be able to take initiative to ensure that service users and their families have the best experience being supported by us, whilst working within set guidelines.

You will receive a full corporate Sirona induction alongside a local induction, where you will receive on the job training. You will undertake regular supervisions with your line manager and actively follow a programme of personal and professional development through setting and regular review of personal development goals. Your development and progression is at the forefront of what we do, we offer opportunities to progress and undertake TNA/AP apprenticeships to become registered Nurses & Therapists.

Our service operates 8am-8pm, 7 days per week. We have a range of full time and part time opportunities available (22.5 hours minimum) and can support flexible working patterns. Evening and Weekend work is required on a rota basis. Driving Licence is essential
