Job Details
We are looking to recruit a Clinical Pharmacist who will work as part of a multi-disciplinary team in a patient-facing role in West 5 Primary Care Network (PCN) Sheffield.
West 5 Primary Care Network (PCN) comprises of 5 GP practices working together with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas, building on existing primary care services to enable greater provision of proactive, personalised, co-ordinated and more integrated health and social care for local people close to home.
Interviews will be via Teams/Zoom on Wednesday 11.8.21
Main duties of the job
- Optimise the use of medicines
- Support the PCN with regards to prescription and medication queries
- Improve the safety and quality of prescribing
- Providing support and education to patients on self-management and ensure referral to appropriate healthcare professionals if required
- Implement and manage the repeat prescribing process for practice and patient requests
- Medication reviews
About us
West Care 5 includes Broomhill and Lodge Moor Surgery, The Crookes Practice, Walkely House Medical Centre, Manchester Road Surgery and Selborne Road Medical Centre, both system 1 and EMIS are used.
We currently have a team of Clinical Pharmacists working in the 5 practices so the successful candidate would work with practice teams but also as part of the Network Pharmacy team

Job responsibilities
Work as part of a multi-disciplinary team in a patient-facing role. Take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy
Provide primary support to general practice staff with regards to prescription and medication queries. Support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice(s)
Support the practice and integrate with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice

Qualifications
Essential
- Mandatory registration with General Pharmaceutical Council
- Membership of the Royal Pharmaceutical Society
- Masters degree in pharmacy (MPharm) (or equivalent)
Desirable
- Member of or working towards Faculty Membership of the Royal Pharmaceutical Society
- Graduate diploma level or equivalent training/experience
- Independent prescriber or working towards/intent of gaining independent prescribing qualification
Experience
Essential
- Minimum of 2 years post- qualification experience
- An appreciation of the nature of GPs and general practices
- An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
Desirable

- In depth therapeutic and clinical knowledge and understanding of the principles of evidence- based healthcare
