Job summary

We are long running GP partnership looking for an enthusiastic Nurse. We are flexible with hours and days. Experience is desirable but not essential.

The post holder will demonstrate courageous and critical thinking and is a caring, compassionate and committed experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process. They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

Main duties of the job

1. To provide a Nursing service resource within the practice.

2. To work within the community, as an autonomous, accountable Nurse, in the provision of a holistic approach for individuals including assessment, diagnosis and treatment, to deliver quality patient services.

3. To assess, diagnose, treat, refer or signpost patients/service users who attend surgery with undifferentiated or undiagnosed condition relating to minor illness or minor injury.

4. The post holder will use advanced clinical skills to provide education to service users, promoting self-care and empower them to make informed choices about their treatment.

Context

The Nurse Practitioner will contribute towards delivering the below listed service / patient outcomes:

Improved and faster access to care for housebound patients at allocated times per week

Improved patient outcomes (including QOF achievement/PMS targets and PCN work)

Prevent deterioration and unnecessary A&E attendances/admissions (especially UTI and Pneumonia admissions)

Improve long term care for house bound patients with chronic conditions

Improved coordination of care for vulnerable and those with chronic disease -Work with quality achievement targets lead to attain this

The Nurse Practitioner

The key focuses of the Nurse Practitioner (ANP) will be:

1. To provide a Nursing service resource within the practice.

2. To work within the community, as an autonomous, accountable Nurse, in the provision of a holistic approach for individuals including assessment, diagnosis and treatment, to deliver quality patient services.

3. To assess, diagnose, treat, refer or signpost patients/service users who attend surgery with undifferentiated or undiagnosed condition relating to minor illness or minor injury.

4. The post holder will use advanced clinical skills to provide education to service users, promoting self-care and empower them to make informed choices about their treatment.

This role includes

The delivery of primary medical care for patients involving:

Triage and treat patients wishing to see a health care professional, making any necessary referrals to other members of the primary health care team

. Be able to perform bloods, smears, ear examination, dressings and wound management, diabetic assessment COPD and Asthma reviews, mental health reviews , learning disability and rheumatological reviews.

.The post holder will ideally be a prescriber and be able to perform minor illness reviews

.The post holder will be able to perform remote reviews to achieve the above whether this be via video or over the phone.

Patients with problems needing referral to secondary care should be discussed with the registered GP if possible, before making such referral.

Ensure clinical practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations.

Advise patients on general health care and minor ailments, with referral to other members of the primary and secondary health care team as necessary

Works mobile conducting home visits for house bound patients, with OHL offices as a base

Works as an autonomous practitioner, in accordance with the Health Professions Council. Ensure that personal and professional clinical standards are maintained.

To undertake assessment for patients, using diagnostic skills and initiation of investigations where appropriate.

To communicate at all levels within the team ensuring an effective service is delivered.

To maintain accurate, contemporaneous healthcare records appropriate to the consultation.

Ensure evidenced-based care is delivered at the highest standards ensuring delivery of high quality patient care.

Works with local policies and procedures.

Enhance own performance through Continuous Professional Development, imparting own knowledge and behaviours to meet the needs of the service.

To achieve and demonstrate agreed standards of personal and professional development in order to meet the needs of the service.

To participate in the audit process, evaluation and implementing plans and practice change in order to meet patient need.

Contribute positively to the effectiveness and efficiency of the team and work colleagues.

Communication

The post-holder should recognise the importance of effective communication within the team and will strive to:

-cmmunicate effectively with other team members

-Communicate effectively with patients and carers

-Recognise peoples needs for alternative methods of communication and respond accordingly

As a member of the team, the post holder may be required to contribute towards the following:

-Communicate with a range of internal and external groups and individuals

-Support the production of audit or and service communications within or outside the practice

-Manage and communicate business sensitive personal information in a secure manner

Person Specification

Qualifications

Essential

  • Registered Nursing or Midwifery Qualification


Desirable

  • Recognised leadership qualification
  • Degree / MSc modules in Advanced Clinical Skills / diagnostics
  • Non-Medical Prescribing Qualification


Experience

Essential

  • Evidence of working in a primary care setting ( GP practice / Walk-in centre / Urgent Care Centre) at a higher clinical level, as an autonomous practitioner


Desirable

  • MSc in Advanced Practice or Clinical Nursing
  • Evidence of post registration CPD to include advanced clinical skills
  • RCN Advanced Practice Credentialing
  • Teaching / Mentoring qualification
  • Experience of work in a general practice team
  • Minimum of 2 years post registration experience
