Job summary

You will join a friendly team working 37.5 hours per week across a 7 day working week with the hours of the service being from 08:00 - 20:00

Main duties of the job

As a Band 5 Rapid Response Nurse within HCRG Care Group Kent your days will be rewarding and varied and include:
  • Enhanced clinical assessment/ decision making and treatment planning to help supports patients safely at home and reduce unnecessary hospital attendance/ admission
  • Collaborative working/ joint visits alongside therapy colleagues within the team to promote holistic person centered assessment and approach to care
  • Ability to apply a problem solving approach when faced with complex situations
  • Support with line management of staff HCAs/ support workers within the services

Contribute towards service development/ in-service training

Main Responsibilities
As a Band 5 Rapid Response Nurse within HCRG Care Group Kent your days will be rewarding and varied and include:

The Ideal Candidate
You will be a qualified, registered Nurse, either as a newly qualified Nurse or already working as a Band 5. You will need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
