Job summary

We are looking for an authentic and emotionally intelligent Team Leader to join and support our primary care team working across HMP Ashfield.

Leadership is a learning journey that never ends; that's why we offer extensive training and development opportunities to help our leaders to be the best they can be and to support their on-going professional and personal progression.

Main duties of the job

Job description

Job responsibilities

You will be employed as a Clinical Team Leader.

You will work as part of the physical healthcare care team, alongside colleagues in mental health and substance misuse, to provide high quality primary care and unplanned emergency response to patients residing at HMP Ashfield.

In the role, you will:
  • Maintain registration in line with requirements of the relevant professional body
  • Participate in the safe and effective delivery of community care services, including but not limited those services listed here: www.hanhamsecurehealth.co.uk/services/
  • Undertake comprehensive assessment of patient needs, including mental capacity and risk assessment
  • Administer medicine, within the guidelines of the relevant professional body
  • Support the delivery of prescribed treatment programmes and/or interventions to patients on the substance misuse caseload; appropriate training will be provided
  • Support patients to manage their own health and promoting self-care
