Experience:
- Supporting adults and/or children with disabilities providing personal care and physical support
Responsibility:
- Completion of outcome focussed support plans and risk assessments which focus on maximising the person independence
- To frequently attend service users' homes to ensure that support and service plans are being delivered in line with service user preferences, wishes & values, and the support plans are adhered to
- Arrange and complete service user care package reviews, to include incidents, health and well-being
- To liaise and establish effective working relationships with staff internal and external, partners and other professionals
- Support in the preparation for the Care Quality Commission Inspection which demonstrates compliance with the regulations and standards
Budgets & equipment:
- Responsible for the correct handling of service user finances and administration of medication in accordance with policy and procedure where applicable and as required
- Collective responsibility for the care and security of equipment and consumables in the persons' home
Information:
- Responsibility for the accuracy, security, and confidentiality of service user records
Communication & Interaction:
- Communication and personal interaction are key features of this role and may include the provision of support to people who use challenging behaviour as a way to communicate their needs and frustrations or whouse alternative communication methods such as pictures and signing systems
- This role also requires participation in and promotion of a team approach at all times, with positive and constructive working relationships for the benefit of the service user.
- Respond effectively to enquiries and to informal customer complaints and compliments and support the resolution of formal complaints in line with Company policy
Working environment:
- The problems faced by many service users may present challenging situations, which may increase the physical, sensory, and emotional demands of the role
- Complete regular workplace inspections and quality audits.
Special Features:
- The role holder will be expected to undergo certified training and refresher courses in a range of areas' including Adult and Children's safeguarding, Positive Behaviour Management, first aid, fire prevention and Health & Safety at Work, in addition to other specialist training related to the nature of the service
- To support the out of hours on-call function on a rostered basis
Apprentices must show commitment to the following:
- To achieve apprenticeship framework (Functional Skills, NVQ and Technical Certificate) within the specified time frame
- Attend the apprenticeship induction programme
- Attend all of the training sessions for the programme whenever necessary
- Undertake any other required development work as part of the apprenticeship programme as requested
- Any other duties as deemed appropriate by senior staff
- Apprenticeships start throughout the year and are open to any age of candidate

- Must be18+ due to the nature of the role
- Desired, Level 2 qualificationin the subject area
- Maths and EnglishGCSE C/4 (or above),functional skills or willingness to work towards
- Must have experience in health care
Future prospects
- The apprenticeship will lead to a permanent contract
