Job summary

An exciting opportunity for a Senior Clinician (Physiotherapist or Nurse) has arisen in the Norfolk and Waveney (East Coast - Great Yarmouth and Waveney area) CCG area. The key purpose of this role is to deliver an NHS commissioned Breathlessness Rehabilitation programme (combined Pulmonary and Heart Failure Rehabilitation). This is a further year extension to an initial one year pilot to incorporate heart failure patients into the existing pulmonary rehabilitation service and will likely receive ongoing funding if it continues to demonstrate good clinical outcomes. Main duties of the job

The Senior Cardiorespiratory Physiotherapist / Nurse supports the Senior Team Lead to plan, promote, co-ordinate, develop, implement, and evaluate a high quality, evidence based Pulmonary Rehabilitation Service. The post-holder will be responsible for the day-to-day management of the heart failure element of the programme to ensure that it delivers pre- and post-assessment, exercise, and education to patients within a group or virtual setting and provides encouragement and support for patient self-management. The post-holder will also be expected to contribute to delivery of the other services provided by the team, such as pulmonary rehabilitation, home oxygen assessment and review (HOSAR) and any other service development projects.

Principal Responsibilities
  • To undertake comprehensive subjective and objective assessment of patients with diverse or complex cardiorespiratory conditions and multiple pathologies. To prioritise patient problems and to formulate and deliver appropriate treatment programmes based on assessment findings, with evidence of discharge planning and in line with national standards. This process is fully autonomous, and treatments will vary according to assessment findings.
  • To evaluate patient progress, reassess and alter treatment programmes if required, based on knowledge of evidence-based practice and patient outcomes. Frequently readjust plans as situations change / arise due to unforeseen circumstances.
  • To lead breathlessness rehabilitation classes delivering a comprehensive programme of exercise and education. Including objectively measured assessment of exercise performance and exercise prescription and progression and assessment, interpretation, and feedback to the patient of initial and final outcome measures.
  • In collaboration with the Team Lead as required, identify patients who meet the national criteria for ambulatory oxygen, provide specialist assessment and highly skilled discussion of complex and highly emotive information to patients. Prescription of appropriate equipment and flow rates will frequently require multifactorial decisions to be made to individualise the prescription to optimise the patient's concordance and usage of the equipment.
  • To demonstrate effective communication and negotiation skills across a range of situations, both clinical and non-clinical
  • To assess risk and manage it effectively within the clinical caseload and community setting
  • To contribute to the overall planning, delivery, strategic development, and evaluation of the Breathlessness Rehabilitation Service in conjunction with other clinical staff and within relevant guidelines and evidence-base.
  • To work collaboratively with other staff in the team and across the Cardiorespiratory landscape to ensure co-ordinated and efficient patient centred and evidence-based care with effective use of all available resources.

  • To assist with the performance management of more junior / support staff and students within the service in conjunction with the Team Lead.
  • To maintain, develop and continually update knowledge, skills, and clinical competency to practice through Continuous Professional Development (CPD) activities to incorporate developments into your work and ensure safe practice while undertaking duties.
  • To be responsible for the effective flow of patients through delegated breathlessness rehabilitation programmes to meet service key performance indicator (KPI) targets and assist the Senior Team Lead by monitoring caseloads, prioritising referrals, collecting, and collating team activity data in an agreed format to monitor the clinical effectiveness of the team.
  • To identify and propose service changes for own work area and participate in appropriate research processes and/or data collection, identified in departmental/rehabilitation objectives, with support from the Senior Leadership team
  • To be responsible for maintaining accurate, up to date and comprehensive patient treatment records (electronic or hand-written) in line with the legal requirements and organisational guidelines showing evidence of clinical reasoning / evaluation and treatments that are evidence-based and within broad professional standards and guidelines (HCPC, NMC, CSP, RCN). In addition, to ensure that students, support staff and any other staff maintain patient records in line with guidance and standards.


Person Specification

Qualifications

Essential

  • Diploma / Degree in Physiotherapy or Nursing
  • Registration with HCPC or NMC
  • Evidence of on-going relevant Postgraduate Continued Professional Development (CPD) in field of Cardiac / Respiratory Care and Pulmonary / Cardiac Rehabilitation,
  • Full valid driving license and access to a vehicle with business insurance


Desirable

  • Level 4 Certificate in Exercise Training / Instruction (ideally BACPR Cardiac exercise specialist or Chronic Respiratory Disease) for Nursing only
  • Post-graduate course in Exercise Physiology / Exercise and Rehabilitation
  • Member of CSP or RCN
  • Member of AACVPR, ACPRC, ARNS, PCRS or relevant SIG
  • First Aid at Work or other 1st aid qualification
  • ALERT Course
  • Spirometry (accredited to ARTP standards)
  • Practice Educator and Assessor / Mentorship Course or equivalent.

Experience

Essential

  • Previous experience at Band 6 or evidence of meeting Band 5 KSF Exit gateways (or equivalent)
  • Clinical Experience of treating patients with cardiorespiratory conditions.
  • Evidence of completing relevant cardiac / respiratory rotation(s) at Band 5.
  • Wide range of acute experience including ICU, medical and surgical, care of the elderly, neurology, orthopaedics, and outpatients.
  • Previous contribution to practice education / training/ support and supervision of less experienced staff / students
  • Experience of working as part of a multidisciplinary team
  • Experience of liaising with other agencies, services, and professional colleagues


Desirable

  • Previous experience of leading or assisting in delivery of Breathlessness / Pulmonary / Cardiac Rehabilitation classes
